1. Acceptance of Terms
These Terms of Use (“Terms”) govern your access to and use of the website cpclinicians.com (the “Site”), operated by Connected Path Clinicians, LLC (“CPC,” “we,” “us,” or “our”). By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.
2. About This Website
CPC is a multi-disciplinary outpatient practice providing speech-language pathology, mental health, behavioral health, and occupational therapy services, independent educational evaluations, and professional training in Delaware and surrounding states. The Site provides information about our services and allows visitors to submit contact, intake, and referral forms and to request appointments through our online scheduling widget.
3. Not Medical Advice; No Provider-Patient Relationship
Content on this Site is provided for general informational purposes only and is not intended as, and should not be relied upon as, medical, clinical, psychological, educational, or professional advice. Use of the Site, including submission of a contact, intake, or referral form, does not create a provider-patient, clinician-client, or confidential relationship with CPC or any of its clinicians. A provider-patient relationship is established only after formal intake, scheduling, and applicable consents are completed directly with CPC.
This Site is not intended for use in a medical emergency. If you or someone else is experiencing a medical or mental health emergency, call 911 or go to the nearest emergency room. If you are experiencing a mental health crisis, you may also contact the 988 Suicide and Crisis Lifeline by calling or texting 988.
4. Contact, Intake, Referral, and Scheduling Forms
You agree to provide accurate and complete information when submitting any form on the Site, including our contact, intake, referral, and scheduling forms. Submission of a form does not guarantee acceptance as a client, availability of services, or a scheduled appointment. We will follow up regarding next steps.
5. Online Scheduling and Payment
Our online scheduling widget, and in the future our online payment widget, are provided through Tebra, a third-party practice management platform operating under a signed Business Associate Agreement with CPC. Your use of these tools is also subject to Tebra’s applicable terms. CPC is not responsible for outages, errors, or issues arising from third-party platforms outside our control.
6. Intellectual Property
All content on the Site — including text, graphics, logos, and the CPC name and branding — is owned by or licensed to CPC and is protected by applicable intellectual property laws. You may view and print content for your personal, non-commercial use, but may not reproduce, distribute, modify, or create derivative works from Site content without our prior written permission.
7. Acceptable Use
When using the Site, you agree not to:
- submit false, misleading, or fraudulent information through any form;
- attempt to gain unauthorized access to the Site, our systems, or any account;
- interfere with or disrupt the Site’s operation, including through malware or excessive automated requests;
- use the Site to harass, threaten, or harm CPC staff, clients, or other visitors; or
- use the Site for any unlawful purpose.
8. Third-Party Links and Services
The Site may link to or embed third-party services, such as Tebra’s scheduling and payment tools. We do not control and are not responsible for the content, availability, or privacy practices of third-party sites or services. Your use of those services is subject to their own terms and policies.
9. Disclaimer of Warranties
THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, OR THAT THE SITE WILL BE UNINTERRUPTED, SECURE, OR ERROR-FREE.
10. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, CPC AND ITS OWNERS, EMPLOYEES, AND CONTRACTORS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF, OR INABILITY TO USE, THE SITE, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. NOTHING IN THESE TERMS LIMITS LIABILITY THAT CANNOT BE LIMITED UNDER APPLICABLE LAW.
11. Indemnification
You agree to indemnify and hold harmless CPC and its owners, employees, and contractors from any claims, damages, or expenses (including reasonable attorneys’ fees) arising from your violation of these Terms or your misuse of the Site.
12. Governing Law and Jurisdiction
These Terms are governed by the laws of the State of Delaware, without regard to conflict of law principles. Any dispute arising from these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Delaware, and you consent to the jurisdiction of those courts.
13. Changes to These Terms
We may update these Terms from time to time. The “Effective Date” above reflects the date of the most recent revision. Continued use of the Site after changes are posted constitutes acceptance of the revised Terms.
