Connected Path Clinicians, LLC (“CPC,” “we,” “us,” or “our”) provides speech-language pathology, mental health, behavioral health, occupational therapy, independent educational evaluation, and professional training services in Delaware and surrounding states. This Privacy Policy applies to information collected through our website and related online tools, such as our contact, intake, and referral forms and our online scheduling widget. It does not apply to protected health information (“PHI”) we collect and use in the course of providing clinical care, which is governed by our HIPAA Notice of Privacy Practices, available at our office and upon request.

2. Information We Collect

Information You Provide Directly. When you use our website’s contact form, intake form, referral form, or scheduling widget, we may collect information such as your name, phone number, email address, mailing address, the name and age of the client or student you are inquiring about, the reason for your inquiry, insurance information, and any other details you choose to include.

Information Collected Automatically. Like most websites, our site (hosted on Netlify) automatically logs certain technical information when you visit, such as your IP address, browser type, device type, referring page, and pages viewed. We use this information for site performance, security, and troubleshooting. As we finalize our analytics and marketing tools, we will update the Cookies and Tracking Technologies section below to reflect the specific tools in use.

Scheduling and Payment Information. If you use our online scheduling widget or, in the future, our online payment widget, information you enter (such as appointment preferences, contact details, and payment information) is collected and processed by our practice management vendor, Tebra, under a signed Business Associate Agreement with CPC. We do not store payment card information on our website or servers.

3. How We Use Your Information

• Respond to Inquiries — to respond to questions submitted through our contact, intake, or referral forms.
• Schedule Services — to process appointment requests submitted through our scheduling widget.
• Process Payments — to process payments you authorize through our payment widget, once available.
• Operate and Improve Our Site — to maintain, secure, and improve the functionality of our website.
• Legal Compliance — to comply with applicable law or respond to lawful requests.

We do not sell your personal information, and we do not use information submitted through our website for marketing purposes without your consent.

4. Cookies and Tracking Technologies

Our website is hosted on Netlify, which may use cookies or similar technologies necessary for basic site functionality and security. We have not yet finalized which analytics or marketing tools (if any) we will use on the site. Before implementing tools such as website analytics or advertising pixels, we will evaluate their privacy and security implications — including guidance from the U.S. Department of Health and Human Services regarding tracking technologies on healthcare websites — and will update this Policy accordingly. Where required by law, we will provide a cookie consent mechanism.

5. How We Share Information

We share information submitted through our website only as follows:

• Service Providers — with vendors who help us operate our website and business, such as Tebra (scheduling and payment processing) and Netlify (website hosting), each of which is contractually obligated to protect your information.
• Legal Requirements — when required by law, subpoena, or other legal process, or to protect the rights, safety, or property of CPC, our clients, or others.
• Business Transfers — in connection with a merger, acquisition, or sale of assets, subject to standard confidentiality protections.

We do not sell your personal information to third parties.

6. Children’s Privacy

Our website is not directed to children under 13, and we do not knowingly collect personal information directly from children through our website. Because many of our clients are minors, information about a child (such as through an intake or referral form) is submitted by a parent, guardian, or referring professional on the child’s behalf, not by the child. If you believe a child has submitted personal information to us directly without parental involvement, please contact us so we can address it.

7. Your Privacy Rights

Depending on where you live, you may have rights regarding the personal information we hold about you. Residents of Delaware, New Jersey, and Maryland — three of the states we serve — are covered by comprehensive state privacy laws that generally provide the rights to:

• Access — confirm whether we process your personal information and obtain a copy of it.
• Correct — correct inaccuracies in your personal information.
• Delete — request deletion of personal information we hold about you.
• Data Portability — obtain a copy of your personal information in a portable format.
• Opt Out — opt out of the sale of personal information, targeted advertising, or certain profiling — we do not currently engage in the sale of personal information or targeted advertising.

We will honor requests to exercise these rights, consistent with applicable law, regardless of your state of residence. To submit a request, contact us using the information in Section 13 below. We will not discriminate against you for exercising your privacy rights. Please note that PHI governed by our HIPAA Notice of Privacy Practices is handled under HIPAA’s access and amendment procedures rather than this section.

8. Data Security

We use reasonable administrative, technical, and physical safeguards designed to protect information submitted through our website. However, no method of transmission over the internet or electronic storage is completely secure, and we cannot guarantee absolute security.

9. Data Retention

We retain information submitted through our website for as long as necessary to respond to your inquiry, provide the services requested, and comply with our legal and recordkeeping obligations, after which it is securely deleted or de-identified.

10. Relationship to Our HIPAA Notice of Privacy Practices

If you become a client of CPC, the protected health information we create and receive in connection with your care is governed by our HIPAA Notice of Privacy Practices, not this website Privacy Policy. A copy of our Notice of Privacy Practices is available at our office and upon request.

11. Third-Party Links

Our website may contain links to third-party sites or services, such as Tebra’s scheduling and payment portals. We are not responsible for the privacy practices of third-party sites, and we encourage you to review their privacy policies directly.

12. Changes to This Policy

We may update this Privacy Policy from time to time. The “Effective Date” above reflects the date of the most recent revision. Material changes will be posted on our website.
