Notice of Privacy Practices

Who We Are

Connected Path Clinicians, LLC (“CPC,” “we,” “us,” or “our”) is a multi-disciplinary outpatient practice providing speech-language pathology services, mental health services, behavioral health services, occupational therapy services, independent educational evaluations (IEEs), and professional training. We provide services in our offices, in schools, in clients’ homes, and via telehealth, to clients and families in Delaware and surrounding states.

Our Commitment to Your Privacy

We are required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice of our legal duties and privacy practices with respect to PHI, to abide by the terms of this notice, and to notify you if a breach occurs that may have compromised the privacy or security of your information.

How We May Use and Disclose Your Health Information

We may use and disclose your PHI for the following purposes without your written authorization:

• Treatment — to provide, coordinate, or manage your care and related services, including consultation between your clinicians and referrals to other providers involved in your care.
• Payment — to bill and collect payment for services, including submitting claims to insurers, verifying coverage, and communicating with payment processors.
• Healthcare Operations — for quality assessment, staff training and supervision, licensing, business planning, and administrative functions necessary to run our practice.
• As Required by Law — including public health reporting, health oversight activities, judicial or administrative proceedings, and reporting related to abuse, neglect, or domestic violence where legally mandated.
• Telehealth — services delivered via telehealth (through Tebra and/or Zoom, both of which operate under signed Business Associate Agreements with CPC) are subject to the same privacy protections as in-person services.

Uses and Disclosures Requiring Your Written Authorization

Other than as described above, we will not use or disclose your PHI without your written authorization, including for:

• Most marketing communications.
• Sale of your PHI.
• Use or disclosure of psychotherapy notes, where applicable to mental health services.

You may revoke a prior authorization in writing at any time, except to the extent we have already relied on it.

Your Rights Regarding Your Health Information

• Right to Inspect and Copy — you may request access to and a copy of your records.
• Right to Request Amendment — you may ask us to amend your records if you believe information is incorrect or incomplete.
• Right to an Accounting of Disclosures — you may request a list of certain disclosures we have made of your PHI.
• Right to Request Restrictions — you may ask us to limit how we use or disclose your PHI for treatment, payment, or operations.
• Right to Request Confidential Communications — you may ask us to contact you in a specific way or at a specific location.
• Right to a Paper Copy — you may request a paper copy of this notice at any time, even if you agreed to receive it electronically.
• Right to be Notified of a Breach — we will notify you if a breach of your unsecured PHI occurs.

Our Responsibilities

• We are required by law to maintain the privacy and security of your PHI.
• We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
• We must follow the duties and privacy practices described in this notice and give you a copy of it.
• We will not use or share your information other than as described here unless you tell us we can in writing; you may revoke that permission at any time, in writing.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.

Changes to This Notice

We reserve the right to change this notice at any time. Any revised notice will apply to all PHI we maintain, including information created or received before the change. A current copy of this notice will always be available at our office and, upon request, in electronic form.
